​Complete this form in order to request funds from the PTO.  You will receive an email from PTO Board letting you know the status.

Please attached file(s) in the "Supporting Files" section for any research you have done on cost and be sure to include enough detail about how the funds will be spent in the "Anticipated Cost(s)" and "Activity/Club Description(s)" sections.   
If you have more supporting file(s) to upload, you may 
e-mail or leave in PTO mailbox.
